The case manager works alongside the individual and their family and/or guardian to ensure the necessary supports. Day Training

Day Training is responsible for serving as a support of participation in daily meaningful routines in the community. It includes regularly scheduled activities in a non-residential setting that are designed to foster the acquisition of skills, build positive social behavior and interpersonal competence, foster greater independence and personal choice. Day Training responsibilities may also include career planning or pre-vocational activities, training and supports designed to maintain skills and functioning and to prevent or slow regression and supported retirement activities.
UP will assist individuals in increasing knowledge, skills and abilities in:

      • Preparing regularly activities in a non-residential setting that will (further) develop skills, positive social behavior and interpersonal competence
      • Developing ways to foster greater independence and personal choice for the individual
      • Developing career planning or pre-vocational activities to develop experitial learning opportunities and career options. These opportunities may evolve by:

        Incorporating person-centered planning designed to support employment-related goals

        Providing active training designed to prepare a participant to transition from school to adult responsibilities, community integration and work

        Assisting & enable each individual to attain the highest level of work in the most integrated setting with the job matched to the participant’s interests, strengths, priorities, abilities and capabilities and includes

      • Skill development to communicate effectively with supervisors, co-workers and customers
      • Generally accepted community workplace conduct and dress
      • Workplace problem solving and skills and strategies
      • General workplace safety
      • The ability to follow directions
      • The ability to attend task
      • Mobility training
      • Develop supported retirement activity plans geared toward the individual
      • Has strong communication skills with diverse populations including; families, team members and the public.
      • Contributes to the professional reputation of the agency.
      • Accepts direction and adheres to company policies and procedures
      • Knowledge of individual rights

Case Management

The case manager works alongside the individual and their family and/or guardian to ensure the necessary supports. As a case manager, we will help individuals and/or their families develop a support plan to meet the individual needs as well as to help them meet their personal and developmental goals. We will stay in close with the individual to keep track of their health, safety and overall welfare. We will also work closely with the individual to make sure they are pleased with the service they are receiving. Services Provided Through SCL or Michelle P.

Community Access

Unlimited Possibilities provides support and training to a participant that empowers the participant to:

1. Participate in meaningful routines and events
2. Hold a membership in a club, group, association, church, business, or organization in the community and
3. Build a natural support system

The service stresses training that assists the person in acquiring, retaining, or improving skills related to independent functioning, self-advocacy, socialization, community participation, personal and financial responsibility, and other skills related to optimal well-being as defined in the Person Centered Plan of Care. These services are designed to result in increased ability to access community resources by natural or unpaid supports. They are provided outside the person’s home or family home

Respite

Respite services are provided to individuals living in their own or family’s home who are unable to independently care for themselves. Respite services are provided on a short-term basis due to the absence of or need for relief of the primary caregiver. Respite may be provided in a variety of settings including the individual’s own home, a private residence or other SCL certified residential setting. Receipt of respite care does not preclude an individual from receiving other services on the same day.

Personal Assistance

    UP will assist individuals in increasing responsibilities abilities related to daily living, help enable a person to accomplish tasks that the person normally would not do for themselves if the person did not have a disability. This will be done through:

      • Provide hands-on assistance (performing a task for a participant)
        Reminding, observing, guiding or training a participant in activities of daily living
      • Reminding, observing, guiding or training a participant in independent activities of daily living
      • Assisting a participant in managing the participant’s medical care including making medical appointments and accompanying the participant to medical appointments
      • Provide transportation, which is not otherwise available under the Medicaid Program, to access community services, activities and appointments
      • Prepare and maintain documentation of services (a note for each contact and a monthly summary note)
      • Has strong communication skills with diverse populations including; families, team members and the public.
      • Contributes to the professional reputation of the agency.
      • Accepts direction and adheres to company policies and procedures
      • Knowledge of individual rights

Behavior Support

UP's Behavior Support Specialists (BSS) provide evidence-based individualized interventions that assist a participant with acquisition or maintenance of skills for community living and behavioral intervention for the reduction of maladaptive behaviors. The BSS is responsible working with individuals supported and their team to develop and ensure proper implementation of behavior support plans. The BSS shall also ensure that the atmosphere of behavioral programming provides optimal growth for the individuals we serve, is based on the person-centered philosophy and preserves their dignity and self-worth. Some of the functions of the BSSs:

  • Conducts functional assessments that meet state regulations and are built upon the concepts of positive behavior supports.
  • Develops behavior support plans and interaction guidelines that utilize ethical, proactive, person centered approaches and meets State standards.
  • Develops methods of tracking effectiveness of behavior support plans.
  • Regularly reviews and analyzes data and make changes to plans as necessary.
  • Conducts person centered planning meetings and Personal Outcome interviews as needed.
  • Promotes self-advocacy and independence by encouraging each individual to identify his/her needs and preferences; and when necessary, provides advocacy on behalf of each individual to ensure appropriate treatment, training and protection of rights.
  • Provides timely and accurate documentation, in compliance with policies, procedures and local regulations.
  • Participates in organizational quality planning, assurance and enhancement processes.
  • Prepare and maintain documentation of services (a note for each contact and a monthly summary note)
  • Has strong communication skills with diverse populations including; families, team members and the public.
  • Contributes to the professional reputation of the agency.
  • Accepts direction and adheres to company policies and procedures
  • Knowledge of individual rights

Community Living Support

Thru Community Supports, UP will assist in helping individuals with intellectual or developmental disabilities increase their independence and promote integration into the community through various events, activities, volunteer work, and outings. ommunity Living Supports assists individuals in becoming empowered to develop natural supports and provide training to the individual in acquiring, practicing, utilizing and improving skills to:

      • Connect with others
      • Independent functioning
      • Self advocacy
      • Socialization
      • Community participation
      • Personal responsibility
      • First Aid & CPR Certification
